A completed and signed verification worksheet is required by the Federal
Government when a student has been selected for verification. A student
must report information about the household size and provide proof of
income by submitting the student and parents prior year tax returns
or other proof of income to enable the Financial Aid office to verify
that the information reported on the FAFSA is accurate. If an asterisk
appears by the EFC number in the upper right corner of the Student Aid
Report, you have been selected for verification by the federal government.
The college may also request verification when needed. If you wish are
required to submit this form, please print this form, sign it and submit
it to the Financial Aid Office at Skyline College.

An increase can be made in a Cost of Attendance budget if documentation
is provided indicating a student must pay for dependent care while attending
college. Complete this form for consideration of an adjustment in your
budget up to $1200.

A Fee Waiver may be available. If you have
completed the admissions process at Skyline, you may use the electronic
application on WebSMART
to apply for the Fee Waiver.
Login to WebSMART. On the Student Services or Registration
menu, click the link to Board of Governors Fee Waiver, complete
the form and submit to see if you qualify for a fee waiver. If qualified,
the Fee Waiver will "waive" the current enrollment fees
and the health fee. If parking fees are greater than $20 per semester,
a Fee Waiver will guarantee that parking charges will be no greater
than $20. The Fee Waiver can be used for any credit class from 1/2
unit up to whatever level of units a student is taking. The Fee
Waiver is only available at a Community College and is available
at any time. The application must be completed annually. All students
are strongly encouraged to complete the Free Application for Federal
Student Aid (FAFSA) in order to receive other grants to assist in
the purchase of books, supplies, gas, and other costs associated
with attending college. |

The chart is a reference to indicate eligibility for a Board of Governors
Fee Waiver strictly based on family size and income. If family income
does not fall within the dollar limits on this chart, the student should
complete the Free Application for Federal Student Aid at www.fafsa.edu.gov.
A student may qualify for a BOGFW-C if there is at least $1 of need. |
